What is a Breast Lift?

Breast lift, or mastopexy, elevates the nipple position on the chest, repositions the breast tissue higher on the chest, tightens the skin around the breast tissue, and generally reduces the size of the areola. All of this will give the breasts a more youthful shape.

What is Breast Augmentation with Lift?

While some patients have breast ptosis, or breast droop, at a young age, many times ptosis occurs after pregnancy or weight loss. Patients with breast ptosis, whose breasts are naturally smaller than they would like, or have lost volume after pregnancy or weight loss, can choose to augment the breast size with implants or fat transfer.

Most often, the procedures can be performed at the same time. However, there are times, generally when patients want a relatively large breast implant, that we will recommend the operation be staged - or done in two operations. We will recommend this when we are concerned that a single operation will compromise the blood supply to the nipple or the tension of the wound closure will likely cause unfavorable scarring.

There certainly are times that I recommend we do the lift and augmentation separately for both safety and aesthetic reasons.

Dr. Lee’s message to patients considering Augmentation Mastopexy: Breasts often need more than just more volume, and we would be doing our patients a disservice by only offering augmentation for breasts that hang too low. Many times the breast tissue and nipple must be repositioned into a higher, more youthful position in order to establish an aesthetic result. The only augmentation that may sometimes avoid a lift in sagging breasts is fat transfer which can distribute volume more evenly throughout the breast tissue. While it may not fix the hanging, it can augment the breasts with a natural appearance and some degree of lift without having breast tissue hang off of a breast implant.

Consultation and Preparation

During your consultation, your surgeon will assess your overall health, discuss your goals, and help you determine the best treatment plan. They may also recommend a mammogram prior to the procedure. To prepare for the treatment, you'll receive detailed pre-operative instructions, which may include stopping certain medications or avoiding smoking.

Choosing an Augmentation Size

Your surgeon will help you determine the appropriate breast implant size based on your aesthetic preferences, your body proportions, and existing breast tissue. You can then discuss the pros and cons of implant versus fat augmentation. Your surgeon may recommend that your augmentation mastopexy be staged, or performed as two procedures.

Procedure

The procedure is a day surgical procedure, you go home about an hour after the procedure, and typically takes between two to four hours, depending on the complexity of the case. Patients are given general anesthesia and monitored during the entire operation. The degree of breast ptosis and type of mastopexy will dictate the incisions used. The implants are placed using the mastopexy incisions.

Breast Ptosis Classification

Breast ptosis (or sagging) is classified, or graded, based on the location of the nipple, nipple relationship to the inframammary fold (IMF, bottom of the breast), and nipple relationship to the breast itself.

Grade 1 Ptosis: The nipple is at the level of the IMF and above the lower pole of the breast.

Grade 2 Ptosis: The nipple is below the level of the IMF and above the lower pole of the breast.

Grade 3 Ptosis: The nipple is below the IMF and at the bottom of the breast (the nipple is the lowest part of the breast).

Pseudoptosis: The nipple is above the IMF, but a disproportionate amount of the breast is below the nipple and below the IMF.

Types of Mastopexy

Crescent: A crescent of skin above the areola is removed. It moves the nipple minimally and is rarely used.

Donut: Incision all around the areola to reduce the size of the areola and remove skin circumferentially around the areola. Useful in cases of large areola and mild or no ptosis.

Lollipop: The incision is all around the areola and then a straight line from the bottom of the areola toward the IMF. It reduces the size of the areola and is used in patients with mild or moderate ptosis.

Anchor: The lollipop all the way to the IMF plus a horizontal incision within the IMF. They are used in moderate and severe ptosis.

Results, Recovery, and Aftercare

After the procedure, you'll need to wear a surgical bra or compression garment to support your breasts and minimize swelling. Expect some downtime and restrictions on activities for a few weeks, and follow your surgeon's guidelines for incision care and post-operative appointments. If you have chosen fat transfer augmentation, you will also follow the post-operative liposuction protocol, which includes a compression garment for a month and lymphatic massages for a month.
